Adaptec AVA-1505 SCSI controller card for ISA slot. This controller allows use of 50-pin SCSI hard drives and other SCSI devices. It does not have a ROM, so I believe it cannot boot directly from hard drives (unless your motherboard's BIOS supports it). Unfortunately, I do not have the rear bracket.
