Job Summary: We are recruiting a Social Worker to provide psychosocial support and follow-up to patients and their families, promoting their social reintegration. Key Highlights: 1. Multidisciplinary team collaboration and professional ethics **Continuous Care Unit**, located in Leiria, seeks to recruit a Social Worker under a full-time individual employment contract. **Role:** Ensure appropriate patient and family support, emphasizing the relevance and centrality of psychosocial factors as determinants or facilitators of treatment, rehabilitation, readaptation, and reintegration of patients/dependents into the most suitable and supportive social environments, fostering the development of all their potential. **Among other responsibilities, the following stand out:** \- Reception \- Development of the individual care plan \- Psychosocial support \- Preparation of care continuity, whenever necessary \- Participation in multidisciplinary meetings (when required). **Requirements:** Bachelor's Degree / Integrated Master's Degree in Social Work (mandatory); Experience in Continuous Care Units and/or training in Gerontology (*preferred*); Registration with the Portuguese Order of Social Workers; Relevant complementary training; Good computer skills and proficiency in Microsoft Office tools (*Word*, *Excel*, *PowerPoint*). **Competencies:** Professional ethics; Responsibility; Analytical ability; Autonomy; Empathy; Patient orientation; Team spirit; Strong communication skills; Type of position: Full-time
